Complete/Yearly Eye Exams or New Patients:
- Chief complaint
- Update/add all meds, allergies and medical history
- Ask if patient wishes to update glasses Rx ($75 fee)
- Vision
- Refraction (as needed)
- Check angles and pupils
- Tonometry (Goldmann or I-care)
- Dilate
- Testing (as needed)

4 to 6 months for glaucoma follow up
- Chief complaint
- Update meds, make sure glaucoma drops listed are correct and verify if patient needs refills
- Vision
- Tonometry (Goldmann)
- Testing (as needed)
**Patients that follow up glaucoma regularly USUALLY are seen 3 times a year. On one visit patient is dilated, on another visit patient has pressure check with NFA & PACHY, and the third visit is usually for Visual field. Double check with patient’s chart or with the doctor to see which appointment is due at that time**

3-6 months for plugs exchange
- Chief complaint
- Update meds, make sure dryness drops listed are correct and verify if patient needs refills
- Vision
- Tonometry (Goldmann or I-care)
- Consent for plugs
- Make sure you leave forceps and plugs in the exam room

BCL removal/replacement

VSP Exams/CTL exams
- Autorefraction (if no CTL)
- Chief Complaint
- Update meds, allergies and medical history
- Vision
- If patient is wearing CTL bring the doctor to the room to check CL Fit and open contact lens template
- Refraction
- Motility test
- Tonometry (Goldmann or I-care)
- Dilate (when needed)
**For established patients we usually have CL information from previous exams. For new patients, make sure you ask them if they know the brand and Rx.
